Provide nursing assessments/interventions. Develop in conjunction with the participant, caregiver, and case manager the Individual Treatment Plan. Coordinate needed medical services, such as; toileting, bathing, mobility assistance, transferring, and other cares as needed. Monitor MD orders. Provide education to center staff, participants and caregivers on health issues.
